I found this great study linking acne to diet. More specifically, foods that spike insulin levels can increase your androgens (male-like hormones) and trigger acne. Terms that are synonymous to high insulin levels include: high carb diet, refined sugar, glycemic load, glycemic index, type 2 diabetes, insulin resistance, metabolic syndrome, syndrome X, and hyperinsulinemia. Are All Benzoyl Peroxides the Same???

Not all benzoyl peroxide (BPO) products are the same. What you need is a stable, water-based BPO for maximum pore penetration. How do you know if it's stable? You don't. Some products oxidize quicker than others. You can tell when this happens because it will change color and become darker just like an apple turns brown when you cut into it.  BPO is typically inexpensive cashing out around $10 to $15 bucks for an ounce. Buy yourself a quality BPO in a small container (1 oz).

You wake up one morning with a raging zit.  Desperate to kill this new growth on your face, you reach into the cubbord for the one thing you are sure will bring death to anything lurking beneath the skin - rubbing alcohol. STOP!!! I beg you, don't ever do this again!  Aside from hardening the tissue, it completely strips the skin of oil. Over drying the skin will cause you to break out even more.  In an attempt to maintain homeostasis, that fine balance where everything continues to function the way it's supposed to, your oil glands pump out even more oil to compensate for what's been irradicated by the alcohol. The best thing you can do is clean it with a gentle, nonabrasive cleanser without rubbing or scrubbing it. Rubbing and scrubbing irritates the follicle and will only make it worse. Next, ice it! Sixty seconds of icing morning and night will reduce the inflamation and help prevent future scarring. Third, apply toner. Toner (without alcohol) removes residual cleanser and lowers the pH of your skin so that the next product you put on penetrates the skin instead of sitting on the surface doing nothing.  Next, apply an exfoliating serum (glycolic, salicylic, lactic, or mandelic). On a pain scale from 1 - 10, it shouldn't be higher than a 2 or 3 when you apply it to your skin. If it's higher than a 3 - that means it's too irritating for your skin (either too strong or the wrong serum).  If you are already using benzoyle peroxide (BPO), you can apply it after the serum. BPO is a very effective product in managing acne when used correctly.  There is a very specific way to use this product - using it incorrectly can make you breakout.

Treating acne is different for every person.  It depends on how much oil you produce, what type of breakouts you get, the location of your breakouts, and the sensitivty of your skin.  All these factors need to be taken into consideration when deciding which acne products to use. What Does Hair Have To Do With Acne???

Hair growth has everything to do with acne! There are two types of follicles, sebaceous follicles (oil follicles) and hair follicles. Attached near the base of both types of follicles are sebaceous glands (oil glands), which resemble a cluster of 4 to 5 grapes. While the hair follicle and the sebaceous follicle both produce oil, it is only the hair follicle that contains a thick hair stalk. The hair acts as a wick for the oil to climb up out of the follicle and onto the surface of the skin.  The sebaceous follicle, on the other hand, may contain a vellus/rudimentary hair at the very most, which is fine, short, and barely visible to the naked eye.  Rudimentary hair presents a problem in acne prone individuals because it either coils up in the follicle, or it is too flimsy to support the weight of the oil - so the oil remains stuck in the follicle along with cellular debris causing an impaction (clogged pore).  Using the correct products and treatments for your specific type of acne will promote healthy sebaceous follicles, and help control your oil and your breakouts.

Does Dry Skin Cause Acne???

No, dry skin does not cause acne, but it can trigger breakouts.  People who have oily, acneic type skin often avoid moisturizers and sunscreens out of fear that it will feel greasy, and potentially cause them to break out.  On the contrary, even oily skin needs hydration.  When we talk about hydrating your skin, we are talking about water.  How do we get water into the skin and keep it there? 

A little dryness can be a good thing under the right circumstances. Dryness leads to the sloughing and flaking  of dead skin cells, and stimulates new cells to move to the surface. If you were to look at a cross section of dry skin underneath a microscope, you would see tiny little cracks called microfissures. These fissures allow water to escape from the skin leaving it dehydrated, and potentially triggering breakouts.  While dry skin can be beneficial in the healing process, it is important not to let it get too dry, and to always keep it hydrated.  First, drink plenty of water - at least half your weight in ounces (more if you exercise or drink coffee).  Second, use a noncomedogenic moisturizer, sunscreen, and/or hydrating serum.  The correct moisturizer or sunscreen sits on top of your skin and creates a seal - locking in moisture.

What Causes Acne? Part 4

Acne vulgaris stems from a genetic predisposition to the disease. The third factor involved in the disease pathway of acne is an oxygen hating microbe, Proprionibacterium acnes (p. acnes).  P. acnes is a bacterium that thrives in anaerobic conditions, making a closed comedone the perfect environment for it to flourish.  Inside the follicle, P. acnes stimulates the production of proinflammatory mediators that pass through the follicle wall activating monocytes and neutrophils (white blood cells), which in turn, leads to the production of proinflammatory cytokines.  A sensitivity to this bacteria (P. acnes) may explain why some acne sufferers experience inflammation with their breakouts and others do not.  As a reminder, a genetic predisposition remains the underlying cause of acne vulgaris, while P. acnes is one of several factors in the disease pathway.

While genetics remains the underlying root cause of acne, the disease process is multifactorial. Leaving off from our last blog, an additional factor involved in the pathogenesis of acne vulgaris is the production and excretion of sebum (oil). It is widely accepted that sebum is controlled by androgens, even though blood tests in numerous acne patients reveal normal androgen levels.  It has been hypothesized that sebaceous glands are not only regulated by androgens, but that other molecules including growth hormone and insulin-like growth factors may also responsible for the production and excretion of sebum, and in turn, the manifestation of acne.

What Causes Acne? Part 2

Although the underlying root cause of acne vulgaris is a genetic predisposition, the disease process has several components; one of which, is retention hyperkeratosis (the rapid production of skin cells retained within a follicle). The follicle we are referring to is what many people commonly know as a pore. It contains sebaceous glands (oil glands), and is lined with epithelial cells (skin cells). There are several popular hypotheses, which help us to understand what causes the acceleration of desquamated skin cells to build up within the follicle, creating follicular plugs.

The three hypotheses of retention hyperkeratosis have one common thread. Androgens. Androgens, are commonly known as male hormones such as testosterone, dihydroepiandrosterone (DHEA), dihydroepiandrosterone sulfate (DHEA-S).

In acne prone individuals, androgens initiate the process of retention keratosis. In addition, girls and boys may begin developing comedonal acne (plugged follicles) as early as age 6 (this is due to ongoing development within a region of the adrenal glands responsible for producing specific androgens). There is a correlation between the degree of comedones in prepubescent girls and their circulating DHEA-S levels. Finally, those with malfunctioning androgen receptors within their sebaceous glands are less likely to develop acne.

While the hypotheses give us insight, the exact cause of retention hyperkeratosis is unknown. Retention hyperkeratosis is one of several factors in the disease pathway of acne vulgaris.

The sole underlying cause of acne lies within your genes. Although, androgen imbalance, adrenal overload, dysbiosis, leaky gut, food sensitivities, chemical sensitivities, and poor detoxification may all be contributing factors, the root cause of acne is a genetic predisposition.  Acne Vulgaris is a dominant trait that may skip a generation. If both parents had acne, three out of four children will also have acne. However, if only one parent had acne, then only one out of four children will have the disease. While acne patterns have a tendency to be similar within families, this is not always the case.

Acne vulgaris is the most common disease in the United States, affecting 60-70% of Americans at some point during their lives. Of this percentage, 20% will suffer severe acne, resulting in scarring and psychological damage. Acne vulgaris affects the face, upper chest, and back due to the abundance of sebaceous follicles (pores containing oil glands). Acne vulgaris is characterized by both noninflammatory and inflammatory acne lesions. Noninflamed lesions include open or closed comedones (blackheads or whiteheads). Whereas, inflamed lesions include papules, pustules, or nodules (pinheads/pimples, superficial pimples with a puss filled cap, pimples with a puss filled cap that extend into the dermis or subcutaneous layer and are greater than 5mm in diameter). The underlying root cause of acne vulgaris is a genetic predisposition towards the disease.


Acne vulgaris is classified more specifically as Grades 1-4 acne. Grade 1 Acne is characterized mainly by multiple open comedones. Grade 2 Acne is characterized mainly by closed comedones, and possibly a few papulopustules. Grade 3 Acne consists mainly of inflamed papules and pustules, but comedones may also be present. Grade 4 Acne is characterized by nodules. Papules, pustules, and comedones may also be present in Grade 4 Acne.

Acne Tips from the Drs.

Today, on the TV show The Drs, one of the physicians recommended salicylic acid and benzoyl peroxide for treating acne. 916 Acne, Skin & Wellness Studio is an Acne Clinic in Northern California. At 916, we prefer to do serum testing on all of our patients prior to making recommendations as some individuals have sensitivities to certain acne products. The type of serum you use to treat your acne not only depends on your sensitivity level, but on your skin type and acne type. Some people cannot tolerate salicylic acid, even at lower concentrations, and do better on a mandelic, or ironically a glycolic. Benzoyl peroxide has proven extremely beneficial for many individuals, but because of its tendency to overly dry and irritate the skin, there is a two week progression when beginning this product, that increases its efficacy and decreases acne flare ups.

Benzoyl Peroxide for Spot Treating!!!

916 Acne, Skin & Wellness Studio sells 10% Benzoyl Peroxide in Chico, California. We also carry 5% and 2.8% BPO. For those that get an occassional pimple, spot treating with 2.8% BPO will do the trick in drying up your pimple, but anything higher than that, and you run the risk of irritation and a secondary breakout from being overly dry. If you have inflamed acne, Benzoyl Peroxide is most effective when it is used as part of an entire home care system... every single day. By using it this way, you can prevent future breakouts. Benzoyl Peroxide is very drying and irritating. For best results, it should be introduced slowly and progressively.

Another Reason Not To Pick!!!

Even the mildest form of acne can cause scarring if you pick. Remember, inflammation leads to Post Inflamatory Hyperpigmentation scarring (PIH). This means that even someone that has noninflamed acne can have PIH. How so? Picking at the non-inflamed lesion, whether it be a blackhead or a whitehead, causes irritation and inflamation, which can lead to PIH. The best way to avoid this type of scarring is...do not pick. Why You Shouldn't Pick!

Picking can cause breaks in the follicle (pore) wall. If you have a pimple or a cyst, it is almost certain that you will cause a break in the follicle wall if you pick. This is BAD! Bacteria from inside the follicle get released into the otherwise sterile environment of the dermis (deeper skin layer). White blood cells begin breaking down collagen and elastin in order to get to the site of infection, and gobble up the bacteria. Can I Get Rid of PIH?

Yes! There are highly effective treatments for Postinflammatory Hyperpigmentation (PIH). Light therapy as well as topical bleaching creams and gels have both proven highly effective.

Bleaching creams and gels are relatively inexpensive compared to light therapy. Bleaching agents even out your skin tone by lightening the darker pigmented area of skin. It does not lighten or whiten your normal skin tone as many believe. Bleaching agents have the potential to physically transform your skin giving you a healthy, even skin tone. Subtle results can be seen as quick as 2 weeks, but may take up 3 to 4 months for optimal results. Although there are many lightening agents on the market, in my experience, I have found that my patients benefit most from a 2% or 4% hydroquinone. I patch test patients first because some people are allergic to this ingredient. Occasionally darker skinned individuals have an adverse reaction to hydroquinone, which gives there skin an ashy appearance. If this should happen, stop using the product immediately.

The ever popular photofacial is a light therapy that evens out skin tone by breaking up the clusters of pigment in your skin. The photofacial/IPL uses an intense pulsed light (IPL) that targets and scatters brown and red pigment in the skin. The scattered pigment is both absorbed by the body and sloughed off with dead skin cells giving your skin a youthful, even appearance. The result is well worth the temporary discomfort. IPL treatments range from $400 to $800 per facial treatment depending on where you go. PIH often require a series of at least 3 treatments depending on the depth and severity of your scarring. For those of you that don't need a full face treatment, 10 minute IPL sessions are also available and usually cost $100 to $150 per session.

Postinflammatory Hyperpigmentation (PIH) is a darkening of the skin in response to inflammation. PIH can occur in the epidermis (upper layers of skin) or the dermis (deeper layers of skin) due to inflammatory molecules stimulating the production of pigment. Does Using Ice Really Help???

Does using ice really help???
Absolutely!!! Of course, it depends on the type of acne. Dr. Fulton refers to ice as the "secret sauce" for those with inflamed acne. If you have pimples, pustules, or cysts with a red base, or any kind of redness from inflammation, than ice is your best friend!!! Inflammation causes scarring. If you put out the fire you will reduce your chances of scarring. Don't skimp or skip when it comes to icing!!!

I suggest my clients ice their acne twice a day applying 60 seconds per area. For example, forehead 60 seconds, one cheek 60 seconds, one side of your jaw 60 seconds, and so on.
You can ice 4 to 5 times a day if you have the time. Ice should be used before you apply your product. If you're going to ice more than twice a day, make sure to wait a couple of hours after applying your product so that you don't dilute it, or wash it off all together. Always make sure to reapply your sunscreen!

The Magic School Bus of Non-Inflamed Acne

Pretend you're travelling across the oil-slicked surface of someone's skin and magically into the belly of a pore (follicle) - it's a long canal lined with dead skin cells, an oil gland beneath you, and a hair that extends out of the follicle like a bean stalk. But, this is no ordinary follicle, this is an impaction, a non-inflamed comedo (pl: comedones) that is rapidly undergoing retention hyperkeratosis. The cells lining the follicle wall are storming down like giant snow flakes so fast they stick together and you're surrounded by a massive glob of dead cells, bacteria, and sebum (oil). Just when you think you will be buried alive by this giant snow ball, you see pac-man like creatures (enzymes) gobbling up the dead skin cells. And, although the snow keeps rapidly falling, the storm is kept at bay by these hungry, protein obsessed, guardians. Although there is a tiny opening at the top of the follicle, it is too small for you to escape, and you are trapped with the impaction inside the follicle.

This is what is known as a "closed comedo" or "whitehead." This is how an impaction develops, and it can take up to 3 months for a comedo to form. Although there is sebum acting as the glue holding this glob of skin cells together, oil continues to glide up the hair, through a small opening, and onto the surface of the skin. Inside the follicle, the oil doesn't become hard like so many of us believe. It appears to be hardened oil when it is expelled, but really, it's just a glob of dead skin cells, a little sebum, and bacteria... a snow ball.

Where were we? Oh yeah, trapped inside the follicle... as the cells continue to storm down and the pac-man-like-creatures (protein digesting enzymes/proteases) get full, the opening at the top of the follicle starts to expand letting in oxygen. The glob of dead skin cells becomes oxidized, and like an apple turns brown when it's exposed to air, so does the impaction. The follicle that you are trapped in is now called an "open comedo" or a "blackhead" and your escape is getting closer. As you and the impaction move towards the opening, little flecks of melanin (pigment) begin to accumulate along side you contributing to the dark color of the blackhead. This is often misconstrued as flecks of dirt from the days activities. Luckily for you, this person decided to get an acne treatment. As the skin care specialist applies pressure at the base of the follicle, you and the impaction, come gliding to the surface, and you high tail it out of there in your magic school bus!

Inflamed versus Non-inflamed Acne

Just as animals, plants, and fungi are organized into different groups so that we can understand them better, so is acne. At the topical level, there are four grades of acne, but to simplify things they have been organized into two general types: inflamed and non-inflamed. Sometime people have a combination of the two types of acne.

Inflamed acne may show any of the following characteristics: redness/blotchiness, hot to the touch; pimples, pustules, whiteheads, or blackheads and possibility of cysts or nodules; excessive oil; skin is sensitive and reactive to products.

Non-inflamed acne may show any of the following characteristics: dull, sluggish appearance; blackheads/bumps, some pimples or pustules; excessive oil; skin is generally not sensitive or reactive to products.

Acne can be controlled with the right acne treatments and combination of products specifically chosen for your type of acne and skin. It requires routine communication to closely monitor your progress, and to make any necessary changes in your home care regimen along the way. Will A Toner Help My Acne?

You just washed your face an hour ago, and you're already sporting the 9am shine! Sometimes we get so desperate to degrease our skin we reach for the strongest stuff we can think of - thinking this ought to do the trick! After doucing your skin with harsh toners, witch hazel, and maybe even alcohol, your breakouts get even worse. Well, not only do these "strippers" degrease your skin, they disrupt the acid mantel, which is partly composed of lipids (oil) that act to protect your skin from foreign invaders and dehydration. When you take this extreme measure, which often many do, your skin begins over compensating by producing even more oil and potentially leads to bigger breakouts.

While toners are important piece of most acne skin care systems, all to often, the over the counter products contain harsh chemicals that are too drying for your skin. A good toner is one that is noncomedogenic, will remove excess oil without stripping, and lowers the pH of your skin to enhance product penetration. This means that the correctives (hydroxy acids, anti-inflamatories, anti-bacterials, bleaching agents, etc.) you apply after your toner will penetrate deeper into your skin and work more effectively.

Some toners contain hydroxy acids, which can be an added benefit...IF...it's suited for your skin type and acne type. A strong glycolic toner could be disastrous for someone with inflamed acne. Likewise, a salicylic toner may remove too much oil for someone who has dry skin. On a scale from 1 to 10 (10 being "holy crap this burns - take it off!!!") your toner should range from a 1 to a 2. Anything higher than that is most likely too irritating to use on a daily basis.

If they sense you are worried or overly concerned, you may be giving them more reason to feel bad. So, continue in your loving way as parents do, and think positive thoughts!

I did think about heating up water on the gas stove to wash my face, but it never actually happened. I DID, however, do my skin care routine every morning and every night, otherwise, my follicles would have filled with oil and stretched to size of saucers. Sitting cross legged on my Paco Pad... no tent for privacy... I used lots and lots of my lactic toner on cotton pads to remove the oil and dirt from my face. I kept using it until no more dirt showed on the cotton pad and there was a small pile stacked in front of me. Next, I did all my usual steps, except in the morning I made sure to use a sunscreen that had 18% zinc oxide. During the day, I would wipe my hands off with a "wet wipe" and reapply more sunscreen. There are "wipes" on the market that are especially for the face (Dermalogica) because they have no irritants and are non-comedogenic. A slave to my skin care routine, it's true, but I had no breakouts and no sunburn. My friend, on the other hand, with his tight little tiny pores, didn't wash his face the entire time!!! And his skin looked beautiful!!!

I just spent my winter break camping out in the middle of the dessert for 8 days. There was no bathrooms, no running water, very little cover from the sun and dust, and temperatures near freezing. I was mainly worried about breakouts and sun exposure because I knew I might be lazy and not wash my face for days, and, I have vampire tendencies where even a minute of sun on my face will make me burn and freckle.
