Benzoyl peroxide is commonly used for the treatment of acne. It is a powerful oxidant that diffuses into the follicles (pores). The action of benzoyl peroxide is similar to that of hydrogen peroxide when applied to a wound. However, it is the benzoyl portion of the molecule that moves peroxide down into the follicles where it kills bacteria and peels the inner lining of the pores. Acne bacteria (P. acnes) are anaerobic and cannot survive in the presence of oxygen. Benzoyl peroxide also works superficially to peel dead surface cells, thus loosening impactions.

Not all benzoyl peroxides are created equal. Certain preparations make benzoyl peroxide ineffective. Nodular and cystic acne are often confused because they look and feel similar. Nodular acne consists of large, hard, painful bumps which are typically inflamed.

A nodule forms when there is a tear or a break at the bottom of the hair follicle (inside the pore) allowing the contents of the acne lesion to drain into the deeper layers of tissue known as the dermis. The once sterile environment of the dermis, now becomes contaminated with oil, bacteria and cellular debris, which triggers an inflammatory response and an infection follows.

Inflamed acne with the development of cysts. Grade 4 acne may present the entire spectrum of acne lesions including balckheads, whiteheads, papules, pustules, nodules and cysts, but the defining characteristic in Grade 4 acne are the presence of cysts. The formation of canals and the joining together of cysts may also be present. This type of acne can become hot to the touch, severely red and inflamed, even bordering on purple.

Those with Grade 4 acne should avoid anything that irritates the skin including the softest brushes, mildest scrubs and gentlest wash cloths. Cleansing should be done with an antibacterial cleanser using only the fingers and patting dry with a clean soft towel. Cystic acne responds well to ice, anti-inflammatories, antibacterials, and exfoliants that do not abrade or create heat on the skin. Inflamed cystic acne responds well to an acne treatment plan, both at home and clinical, that is strong yet gentle.

***GRADE 3 ACNE:

Grade 3 acne is classified as inflamed acne, where the dominant lesions are inflamed papules and pustules, but there may or may not be whiteheads and blackheads present.

Grade 3 acne responds best to antibacterials, anti-inflammatories and gentle products that will not exacerbate inflammation.  For this reason, Clarisonic-type brushes, scrubs, wash cloths, anything abrasive or products that feel warm/hot on the skin should be avoided. Icing the inflamed acne areas twice a day will help reduce the inflammation, and will help prevent scarring and post inflammatory hyperpigmentation (PIH).

As with other grades of acne, Grade 3 acne can also vary in severity. Even severe cases tend to respond quickly when the proper acne treatments are administered and acne products for home use are selected on a per case basis.

GRADE 2 ACNE:

This is the most difficult type of acne to treat. Grade 2 acne is a non-inflamed condition. Although there may be several inflamed pimples present, the majority of the acne lesions are noninflamed whiteheads. Whiteheads that do not mature into blackheads are technically classified as an acneic condition called maturation arrest acne; where the opening of the follicles remain closed, and the contents of the whiteheads are thus trapped inside the follicles. This type of acne often requires massive exfoliation using different treatment modalities in order to open the comedones and clear the acne impactions. Grade 2 acne that is not accompanied by inflamed pimples or pustules can typically tolerate the Clarisonic Face Brush, scrubs, hydroxy acids and retinoic acids for home care.
