Botox Treatment for Acne

A analysis was conducted by Anil Shah, MD, FACS using an off-label Botox procedure to monitor the safety and effectiveness of botulinum toxin on sebaceous (oil) gland activity and pore size in 20 patients. Typically, for cosmetic procedures, Botox is reconstituted with 2cc Bacteriostatic saline per 100 units of Botulism toxin. For the purpose of this study, however, Botox was reconstituted with 5cc Bacterostatic saline - yielding a more diluted product than what would be used for typical cosmetic procedures. This is important, I don't want people out there to read this and have an image of frozen, motionless faces. With a very tiny needle (30 gauge or smaller), Microdroplets of the Botulism toxin were injected into targeted areas of the dermis.

After one treatment, the one month follow-up appointment revealed that 17 out of 20 patients experienced a decrease in oil production, and 17 out of 20 patients experienced reduced pore size.  Six out of 20 patients experienced minor redness around the injection sites at the inner cheeks and the nose for approximately 3 days.  None of the patients experienced facial weakness. Nicely done Dr. Shah!

What Are Whiteheads?

There are four types of acne lesions (noninflammatory acne lesions), one of which, is the stubborn whitehead. Whiteheads are small white or flesh colored bumps on the skin's surface - grainy to the touch. Whiteheads are considered noninflammatory, meaning the tissue surrounding the the whitehead is not red. Technically called a closed comedone or comedo because the opening to the follicle remains closed. This is due to the accumulation of dead skin cells that block the opening, as well as the smaller nature of this type of acne lesion, which prevents the opening from dilating. Over time, the contents of the whitehead can harden. You've may have experience this when you try to pop a whitehead, and NOTHING comes out, and all you end up doing is bruising the skin and making a mess of your face. This type of acne can be the most stubborn, and requires treatments and products that peel and prepare the skin for extractions.

How To Use A Retinoid For Acne

Many acne sufferers are a little apprehensive about using retinoids because they don't want to deal with the the "ugly phase." The ugly phase equates to dryness, redness, and flaking, which can be quite severe depending on the strength of the retinoid and how often it is applied. However, there is such a thing as avoiding the "ugly phase" by gradually allowing your skin to adapt to the retinoid. Begin by applying your retinoid to cleansed skin every third night for two weeks. If you're not experiencing redness, irritation, or excessive peeling, bump up the frequency by applying every other night for two weeks. Once again, if your skin appears to be tolerating the retinoid, go ahead and use it every night. Remember, different types of acne require different types of retinoids. What is Adapalene?

Adapalene is in a class of retinoids that is commonly used to treat mild to moderate acne. Retinoids  (vitamin A derivative) have many functions, but as far as acne is concerned, retinoids help regulate the growth of epithelial cells, by speeding up the time it takes for new skin cells to reach the skin's surface - thus increasing the rate at which the skin sheds. This process is known as "cell-turn-over." By speeding up cell-turn-over, the skin becomes smooth, decongested, and less prone to breakouts. Approved in 1996 by the FDA to treat acne, Adapalene is available under the brand name Differin in a cream or gel base. Acne or Keratosis Pilaris?

Keratosis pilaris (KP) is commonly confused with acne in Chico.  KP occurs when keratin, a protein that protects skin, blocks the hair follicle (pore). Here are a few hints to help you determine if you have KP.

KP usually occurs in patches. It's texture resembles chicken skin or goosebumps with tiny acne-like bumps that may or may not be red. These infamous bumps are most commonly found on the backs of the upper arms, fronts of the thighs, and the buttocks region.

KP does not generally hurt or itch, and is unresponsive to scrubbing and many over-the-counter products. Mechanical exfoliants such as scrubs or a Clarisonic brush may be helpful to a small degree, but they will not get rid of the bumps. While there are a variety of topical treatments, KP responds well to consistent use of hydroxy acids, specifically lactic or salicylic, depending on the strength.

Results may be seen more quickly when a hydroxy acid is combined with a retinoid. Because KP worsens with dry skin, it is important that the products you choose are formulated with ingredients that also hydrate the skin.

TIPS FOR CUTTING BACK ON CAFFEINE IN ONE WEEK:  It's best to start this process over the weekend.
Days 1 - 3: Cut caffeine intake in half for coffee, soda, and black tea. If you drink more than one cup of green tea a day, cut your intake of green tea in half as well.
Days 4 - 6: Avoid coffee, soda, and black tea. Drink one cup of caffeinated, yes caffeinated, green tea.
Days 1 - 7: Take 1000mg to 3000mg of buffered vitamin C with Magnesium and Calcium (powdered form recommended).  Drink half your weight in ounces every day to help minimize withdrawal symptoms, stay hydrated, and flush toxins.

The back scrubber/back brush is a must have for anyone suffering from non-inflamed back acne. If your acne is red, pink, or has a red/pink base, it is considered inflamed, and scrubbing over the top of it will only make it worse. The back scrubber is great for blackheads, whiteheads, and anyone that has a grainy texture, or is prone to acne breakouts. The brush can be used wet or dry, every day, or twice a day, as long as it doesn't feel too abrasive. If your skin begins to feel irritated, it's an indicator you are over doing it. So, if you have been scrubbing twice a day, take it down to once a day. If it still feels irritated, take it down to every other day, and so on. The bristles on your brush should be soft - firm bristles are not better. The plastic back scrubbers with synthetic bristles are less likely to harbor bacteria.
